The online monthly reporting form is now live and ready to be utilised to track incidents and generate performance reports. The form will allow users to seamlessly access the form through their phone or computer and report incidents from anywhere.
How to access the form?

This should load the authentication page as seen below.
Authentication
STEP 1: Enter an active email address and click on Get Access Code
STEP 2: You will receive an email with the authentication code
STEP 3: Input the code received and click on Get Access
The Service Logic – Accident Summary dashboard will load. You will see a list of previously submitted forms here(if any) for editing.
STEP 4: To submit a new report click on Add New Accident Summary
This will load a new form.
STEP 5: Submit Report
Click on Finish Inspection after entering all the details to save the form
